Write a treatment plan for one student.

The evidence included on this page represents a treatment plan I wrote, which is required of the teachers to write for certain students. Every month the teachers must write up an academic summary with a minimum of 5 sentences, include a P.I. index, and planning notes at the end of the report for a child to be given to the principle. In the school summary they are to identify academic strengths and needs as well as including the behavioral strengths and needs of the students. The P.I. index number describes the progress of the student in the case record. The guidelines for the P.I. numbers are as follows: (1) there was no progress made, (2) minimal progress made, (3) average, (4) above average, and (5) is significant progress. Lastly, incorporating a planning note at the end of the individual treatment plan mentions what type of action will be administered to help the child progress. The treatment note helps to monitor if or when advancement is made for the student and gives an accurate representation of the student's strengths and needs.
In order to create an accurate treatment plan I had to have sufficient interaction with the student in their classes. I needed to see their weaknesses and strengths in a classroom environment, their motivation level, the effort made to understand learning material, and the interactions with teachers. I also needed to focus on what their peer relations were like in and out of the classroom setting. I learned to keep note of their behaviors toward other students, their teachers, their studies, and their overall attitude. I became familiar with the student's academic abilities and behaviors by working with them one on one and when they were set in larger classes. I thoroughly enjoyed learning how to complete a treatment note. It taught me how to adjust and learn how to best help a student who is very low skilled. My listening and observation skills really prompted me to achieve an accurate and helpful description of the child.
